    REGISTRATION
    Once you register, you will receive an e-mail notification that your registration has been received. However, registration will not be complete until your credit card information has been successfully processed. (Should any issues arise in the processing of your payment, you will be notified.)

    CLOSE OF REGISTRATION
    Registration will close on Friday, October 8, 2010, or when capacity is reached; whichever comes first.

    PROGRAM MATERIALS
    As noted above, program materials will be made available online this year two weeks prior to the program (October 4); and will remain online until two weeks post-program (November 1). Please return to this site before October 18 to print out your program materials prior to the symposium. Materials will not be available at the program.

    FEES
    $105 registration fee for all attendees. Fee includes continental breakfast, lunch and CEs. Payment must accompany online registration. Seating is limited.

    CERTIFICATE OF COMPLETION
    Please note that a certificate of completion cannot be issued to participants who arrive more than 15 minutes late or leave more than 15 minutes early.

    CANCELLATIONS / REFUNDS
    Please note that 50% of your registration fee may be refunded upon written request prior to October 8, 2010. Refunds will not be available after this date. In the event that Sutter Heart & Vascular Institute (SHVI) should cancel this program, all paid meeting fees will be refunded. SHVI is not responsible for other expenses incurred, including travel.
